Appointments & Office Info
Advanced Retina & Eye Cancer Center is open weekdays from 9:00 am to 5:00 pm. To schedule an appointment, please call us at 480.397.9560. Because retinal emergencies can happen anytime, our after-hours service will provide access to the on-call physician.
To expedite your check-in process we encourage you to bring your insurance cards, photo identification, and your co-pay to every visit. Our office accepts personal checks and money orders, as well as Visa, Discover, and MasterCard.
First-time patients are encouraged to fill out the forms shown below and to bring all pertinent medical records, test results, and a list of current medications.
As a courtesy to our office and to others who may be in immediate need to see the doctor, we request that you call in cancellations at least 24 hours in advance.
